Yeah it looks pretty old, it says 1989 on it.

Sorry for the bad picture, still need some more cleanup.

Yep a standard 5v DX2-66, the 5v SL typically have 1989, 1992 copyright. You can set most boards as a DX-33 and it should work ok. On some boards with 5v SL variant you have to set those different than a standard DX2-66 even though the SL do not have write back cache (as per Bitwranglers comment and why I mentioned it, have run into it on some later model FIC and other boards...)
