My guess is it's an VGA/EGA/CGA combo card, you can see the feature connector, cga output, and standard four dip switches, but what's with round connector?

It looks like S-Video with the version that has two cables at the end- one for video-out and one for video-in.

Yes, I would bet on an ATI EGA card as well. 9 pin EGA connector, plus the usual dip switches, rca jacks and the mouse connector.

Main problem I see is that ATI VGA wonder series was the first to include the bus mouse connector when it comes to ATI cards if I remember correctly.
