The date code on the PCB is 1603, which I think is too old for a Radeon 9250. Maybe a 9000 though, like brostenen posted.

From a quick web search, the 2 reviews that came up for 9250 cards were posted in 9/2004, and the user reviews I found were from 2005. I didn't look very hard but it looks like early/mid 03 is too early for a 9250.

I had a blue Hercules card, but mine was a GF2MX from 2000. I don't know if they were still using that color in 03.

9250 was launched in July 2004 so you are probably correct. A 9200 is pretty much the same chip and was launched in April 2003.
Radeon 9000 is clocked lower and usually comes without a fan so I suppose it would be a 9000 Pro, 9200 or 9250.
Also I found evidence that C.P. Technology is the same as the PowerColor brand (or was at least at the time).

Like I already said:

On a second note... I think it might be even an older card... maybe TNT M64 or GF2MX.

This is a link to the same card on ebay:

Newer cards have different PCB layout and components (more integrated)... even DVI connector.

Requests are also possible... /msg kixs

Although the ebay description could be wrong I agree with kixs. (Good find btw.) Having no DVI or TV-Out would point more to an M64 than a 9250. 2003 would be rather late for an M64, but I suppose those were still made in 2003.
