The board has VIP Rev A written on it. This is the closest I found that resembles just about all of it with a few different jumpers.

Can you take a good picture of your board and post it here ?

These boards are not the same as yours, but they use the same OPTi 82C895 chipset; the BIOS images may work on yours.

There's an MR BIOS for this chipset you can try out, too.
I can't guarantee that it will work on your board, but if it does it'll be an obvious improvement over the Phoenix BIOS.
