Is it me or the driver settings for Lod bias don't have any influence on dgvoodoo? At least I experimented quite a bit with it and didn't see any difference. And it looks like by default dgvoodoo tends to set lod bias more on the negative side? Cause I'm getting strong moire patterns on mipmaps …

Ok, it's a bug in dgVoodoo, it turned out to be a minor, special incompatibility with D3D9, exploited in the game shaders. I fixed it in D3D12, now it's working. If you change the order of displays in windows settings then you should restart the CPL to detect the changes. Anyway, other applications can use the other GPU (like playing a movie in a browser on the other display) while playing the game. Check out the task manager to see per-process gpu usage.

That's what I get if I start the game on the dGPU at high res and then on the iGPU. The game doesn't find the high res amongst the enumerated ones and quits. Try it with unforced resolution at 4K set from the game. Unfortunately some post-process effects proved to be incompatible with resolution forcing, in other games too. Tried it. Without resolution forcing in game heat haze is not visible/effect is minimum and the aura/ bloom flickers with camera …
