I know this thread is a bit old but I do have one UMC socket 5 motherboard. The config is UMC 8891/8892/8886. Motherboard is a Chaintech 5UBM.

Well, it works for ME and SE. Any reason why you are going FE? Edit: AMD driver says it will work in 98FE too.

I bet the card uses a 3DLabs Permedia 2 chip. It's very similar to other cards, use of SGRAM... Joytech Apollo 3000

Ok, now regarding the hard drive. It was probably some component failure in the HDD. Old stuff, It happens. Don't use it anymore unless you want to risk some fireworks.
