I have a v3 mac card, is there a boxed version of such a card? I can't seem to find pictures of it... To my knowledge, I've never heard of an "official" Voodoo3 MAC card, but I believe 3DFX did release MAC drivers for the V3 PCI but that required flashing of the card's ROM as well.
