Do you have something that allows direct hardware access (motherboard monitors and the such)? GIVEIO.SYS or similar? Search for any files with the string "Ke386" in them. There should only be one or two, and those should both be VGA drivers.

And use the Launchpad. If you are using the Launchpad, do not use the "Enable basic VESA emulation" -- it will cause problems on some configurations (thanks for 'patching' that up Microsoft!)

Try running Monkey Island with the flag for a specific audio type eg. monkey a for adlib monkey r for mt-32

FILES= sets the number of files that DOS will allow to be open simultaneously (8-255). Higher numbers take more environment space so it's always a balance between how many files need to be open at the same time and how much environment space is needed.

Try reducing DMA polling periods, and try reducing DOS CPU usage time. They're both available on the Advanced tab for the VDMSound Launchpad IIRC.
