That is because your Windows installation is broken. I have only seen this occur on systems that have been infected with spyware. To fix, copy C:\WINDOWS\REPAIR\AUTOEXEC.NT to C:\WINDOWS\SYSTEM32. Problem solved.

UAKM is not packed, dear. There are specific instructions for UAKM posted elsewhere on the site. There is only 1 CLI that needs to be changed, in the mouse handling loop. This will fix the problem without causing great instability.

DOSBox just forwards the MIDI (MPU-401) to the host controller (whatever your preferred MIDI device is in your OS). I imagine Bochs does the same thing.

You've probably got your Intel "Extreme" (ha!) set up to have the screen rotated at whatever resolution it is being changed to. Disable rotation in the Intel Extreme Graphics control panel.
