I bought a boxed memory expansion card a while back and it came in yesterday. It's a full height, full length 16-bit ISA card and it will give 640K with 4164s and 2.5M with 41256s. Some boards have a BIOS option to emulate USB keyboards, but I don't think that will help you here. Disks sometimes have media defects. I wouldn't worry terribly much. Other users are giving other possible answers, but with old hard disks I always just expect a few bad clusters, honestly. Even the best, new in-box disks have media defects from the factory.

Doesn't the AST RAMPage 286 support 16 bit access through the SuperPak software? If you're installing option ROMs, do most motherboards of this era not have option ROM sockets on the board? Why not install it on a network card or other device with a socket?

Using an analog multimeter would make it plenty noticeable, though. I've got a 10V range on mine and it will show every little spike in that range. If it pegs the scale on 5V when you turn it off, you know it has that problem.
