Does the machine have another game-port? Look in the BIOS to disable any on-board sound and MIDI options.

Try configuring the game without digital sound, just General MIDI. Do you still get issues? Is there anything connected to the MIDI / Joystick port?

Pull out the sound card and could you then test again? Quake II is the easiest one / most consistent. What FPS are you then getting at 640x480x16?
