Do not try rewrite media. It will fail for sure. Well, I haven't had any problem with my LU005S drives and CD-R. (except I've been that stupid and put them to scrap 23 years ago)

Create a Win95 partition with FAT 32. (Minimum size > 256 MB). Create a DOS partition with FAT 16. Install a boot manager. Or use FDISK to change active partition. MS-DOS partition would be drive D then for Win95.

Well, that are different implementations at different stages. The one I quoted seems promising with NCR/Symbios/LSI 875 adapters. The one you quoted is an incomplete attempt on an Adaptec 1520/1522 adapter.
