If you have a 3D printer, you can print one. I've made some brackets for cards missing the PCI bracket. You can go on Thingiverse and download the 3D model of a standard PCI bracket, then just "delete" or cut out the holes for the card design you need.

Leds don't really die. At least not the old red ones. It's far more likely the jumpers are wrong. Jumpers are correct. I have the schematic / jumper legend for it. LEDs are dead.
