There´s a voltage regulator on your mainboard, so - if jumpered correctly - a DX4 should run with a Vcore of 3.3V. Your BIOS also identifies it correctly. The soldered jumper on the left is in a different position. Maybe that´s to enable different EEPROMs or maybe it´s just a different mainboard revision.

Did you try completely clearing CMOS? (remove battery and shorten the clear cmos jumper) Is there a later BIOS version available?

With my experiences with Epox boards from that era I wouldn´t bother troubleshooting any longer before all the capacitors in the power supply circuits have been replaced. There´s a rating printed somewhere on the fuse. It´s usually something between 1A and 4A. The voltage doesn´t matter. Just look for a "slow blow" type with a form factor that fits best.

Look for "!TRM0001.CFG" it´s easy to find. The board identifies as NAME="EISA-486C SYSTEM BOARD" MFR="TEKRAM" The jumpers /switches are not identified 100% correctly though as far as I remember.

I have this 486 EISA AT board and the ECU demanded a TEKRAM cfg file when I did the configuration. There is very little information about this board to be found on the net but it´s faster than my (similar) ASUS EISA 486E.
