Its a Pro 16III without IDE and without Wavetable. Just OPL as a midi synthesitzer. there are around 15 variations of this card with slight differences.

the pcb markings (behind the pci slots) suggest you can set a jumper in the frontpanel io pins to power the mainboard by a "switch" or "ac-on". there are also pins for a power button. also the db9 connector in the power supply is concerning... but maybe thats there to monitor/control the UPS?

looking at the pinout... I wonder, are any of those pins that were missing really required for the computer to interface with the drive ? The left hole is also very close to two vias. You wouldn't want to accidentally sever or short either of those. It might not be possible to repair this reliably for insertion and removal. But what you can do is cut away part of the protective housing of a connector and solder a bodge wire from from the pins to the PCB. Then, add some hot glue for mechanical support.
