Hmm, I threw away such a case a few years ago. Same format just older, 4-color CGA LC-Display (I kept that one). It had a tandy sticker on it. So not homemade.

It's at least the file PIC_A.BND. Replacing it helped. The last part of it seems to be overwritten with "Microsoft Plus! for Windows"...

Try to launch it from the [autoexec] section instead of the command line.

Start -> programs -> dosbox 0.73 -> configuration -> edit configuration

fixrtmidi was a fix for the existing midi output support, not the input patch.

On some systems deactivating the driver deactivates the hardware, too. Porttalk is a hack. Better do it the proper way with a Windows program.

Porttalk doesn't give you interrupts. Would be error prone under a multitasking operating system anyway.

It goes through the Win32 API in both DOSBox and NTVDM. In the latter Porttalk may only help you if your program uses the registers directly. No interrupt support though. In DOSBox it won't be used at all.
