There's certainly no specific support currently. It has been suggested that a serial mouse may work as a second mouse using direct serial support in DOSBox, but I haven't seen anyone confirm this.

Never did get into the Ultima series. Missed out on the "Master of" and X-COM games as well. Betrayal at Krondor and Tie Fighter are easily in my top 10 PC games of all time list though.

You should be able to get it working by using the headers and libs available on libsdl.org. I was once able to get it working with the files from the DirectX 9 SDK (just renamed the files and put them in the appropriate places), but it was a pain and I have since had to reintall mingw/msys.
