I must admit that I'm in the same camp as Alkarion. I haven't used VDMSound for a couple of years and don't have much interest in doing so.

Try lowering the res, try playing the the fullfixed setting. Try changing the output setting to ddraw or opengl.

For the most part, those modded drivers are actually just the offical drivers but with a different set of default driver configuration settings. I'll be damned, it works! My CD-ROM drive is W:\ so I did this: mount d w:\ -t cdrom -ioctl -label ARENA_CD

I tried it with my MT-32 and thought that I wasn't hearing much because the game expected me to be using a CM-32L. Have you tried running with VDMSound or anything like that?

Hmm that's interesting. It could certainly be related, since I didn't get that error. If you run DXDIAG and bring up the DirectX Files tab, are there any issues listed?

For the most part, versions of DOSBox newer than those known to work with a specific game will still work. There are a few exceptions though.

Underdogs has it. I seem to remember there was either a separate EXE or BAT file for each graphics mode (CGA, EGA, VGA), or maybe it was a command line parameter.

So you actually set up a profile within the utility that points to the game's EXE file, and then you're using the utility to launch the game? I haven't tried it so I have no idea what to do.
