To reflect the lock status in the title bar, I think sdlmain.cpp is the only source file you'll need to work with. An example of manipulating the window caption, though not exactly what you want to do, may still be instructive: Title Bar

What you describe sounds similar to what can happen when the mouse is unlocked, so make sure the mouse is locked. If you have autolock=false, you have to manually lock with Ctrl-F10; but with the default autolock=true you can just click on the DOSBox window to lock.

Sounds similar to an issue in Settlers 2 that was fixed in SVN, so try an SVN build. In Settlers 2, the issue would occur more rapidly by moving the mouse in one direction on an axis (e.g. only to the right and never to the left), which may help in testing the issue in this case.

IIRC, the "scrunched video" problem is related to VESA and can be resolved with a VESA driver (e.g. SciTech Display Doctor) and/or command line switches made available in a patch. Not sure if you need 1.03 in addition to 1.05, but both patches are here.

This seems close to what you're looking for: DOSShark for DOSbox 0.74 You may get some pointers (heh, a dual meaning) from it, but note that relative memory addresses can vary from one build of DOSBox to another.

It appears that the audio status function should report zero start and end times when the device is not playing. It makes sense, and the game wants the behavior in any case. A patch for the issue is attached.
