You must edit a file called dosbox.conf (inside your dosbox folder) with a text editting tool as notepad. Then look for an output=xxxx setting and change it to output=surface (not advisable) or output=overlay. I have found out than the remaining types are somewhat blurry due to bilinear filtering …

Try running without any scaler. Try using output=surface or overlay.

[MOVED] Try disabling GUS (Gravis Ultrasound) in DOSBox configuration file. Try changing SB type just for checking...

What keyboard do you have? you may need to enter a keyboard command to establish country pagecode. For me it's keyb sp at the z:\> prompt or in the autoexec section of the config file.
