Unfortunately the sticker is stuck down tightly, and I currently don't have a 486 VLB mobo up and running. Sorry.

I'm also not sure if software is able to differentiate between the chip variations.
1982 to 2001
PCB layout and printed number can be misleading. These chips were pin compatible and manufacturers quite often didn't bothered to change anything on PCB.

But FCC ID numbers are misleading too. I found exactly same card with what it looks like 9400CXi marking:
