I'm looking for a tool/program that can be used to create an installer for a Windows 3.x program. It mainly needs to be able to create/add a new Program Group during installation.

I can make DOS installers and there's lots for modern Windows like Inno Setup, but I have no idea about making installers for pre-W95 era Windows!

Any help is appreciated (and mods, please move this post if it is better somewhere else).

InnoSetup 1.2.16 is capable of building Windows 3.1 installers.
