I installed and ran this yesterday, the following are my instructions:
Baldur's Gate 1 and 2 Installation and Instructions.
1. Install Baldur's Gate 1 and Tales of the Sword Coast. (Do a custom install with all options)
2. Install v5512 Update for Tales of the Sword Coast.
3. Install Baldur's Gate 2 and Throne of Bhaal.
4. Install v6498 Update for Throne of Bhaal.
5. Execute each game and verify that they work before performing the steps below!
SMM Auto-Installation Installation and instructions:
1. Download SMM Auto-Installation from here:

You want:
MegaMoshi_v0.10a.rar
MOD_SOURCE.TXT
2. Download this MOD:

3. Create a MOD_SOURCE directory and place the the above MOD into it.
4. Download Free Download Manager and install it. (This is necessary to download the MODS)
5. Open Free Download Manager and import "MOD_SOURCE.TXT" to download all of the MODS. Download the MODS to the MOD_SOURCE folder you created above.
6. Not all of the MODS will download. Check the download status and make a note of the MODS that did not download.
You may also want to manually extract each MOD to verify that the downloads are good. You don't want to spend hours installing only to have to restart because of a bad download.
7. Move the MOD_SOURCE folder to your installed Baldur's Gate 2 + TOB directory.
8. Extract the files from MegaMoshi_v0.10a.rar and place them in the Baldur's Gate 2 + TOB directory.
You should now have a "megamoshi.exe in your BG2 directory and a "SMM-patch" folder.
9. Execute MegaMoshi.exe and go to the settings tab. Make sure that the paths to your installed copies of BG1 and BG2 are correct. Also make sure that the path to Winrar is correct, the executable for Winrar must be specified.
EX:
Baldur's Gate Tales of the Sword Coast Installation Path:
I:\BG\MODDED\BG1
Shadows of Amn Throne of Bhaal Installation Path:
I:\BG\MODDED\BG2
Winrar Installation Path:
C:\Program Files\WinRAR\WinRAR.EXE
If you cannot modify the paths in Megamoshi or do not wish to then you can edit the "SMM-CONFIG.XML" file in the "SMM-patch" folder.
10. In your installed copy of Baldur's Gate 1, open the "Baldur.ini" file and make sure that only the following line is present (change the path for your system):
HD0:=I:\BG\MODDED\BG1
Remove the following lines:
CD1
CD2
CD3
CD4
CD5
In Baldur's Gate 2 make sure you change the paths where appropriate:
HD0:=I:\bg\modded\bg2\
CD1:=I:\bg\modded\bg2\CD1\
CD2:=I:\bg\modded\bg2\CD2
CD3:=I:\bg\modded\bg2\CD3\
CD4:=I:\bg\modded\bg2\CD4\
CD5:=I:\bg\modded\bg2\CD5\
11. Execute MegaMoshi and press "Install", it should error out. This is because some of the MODS are incorrect:
Check the "Missing Mods" tab and it will show the mods that it is missing. They are most likely the same MODS that were missing in your downloads.
More than likely the MODS listed in MegaMoshi are outdated and you will need to download the latest versions (and place them in the MOD_SOURCE folder). So download the latest versions of the MODS.
After you do this then you will need to edit the "SMM-CONFIG.XML" file with the names for the updates MODS. It's best to leave MegaMoshi open in one Window with the "Missing Mods" tab, your downloaded Updated MODS in another Windows and "SMM-CONFIG.XML" opened in Notepad in another Windows. Now use the "replace" function in notepad and replace the outdated mods with the new ones. Some mods may have different names and some mods may be self-extracing whereas the previous version use a .zip. Study each new MOD and make it the same as the old MOD.
So if the old MOD was a .zip and the new mod is a self-extracting .exe, then extract the .exe and make it a .zip.
In some cases the new MODS also use new setup names. So if the old one was setup19.exe then the new one would be setup20.exe. You should note this and check the "SMM-CONFIG.XML" file when you are updating it.
Once you are finished and all of the names are updated then when you press "Install" in MegaMoshi it should begin installing without errors!.
12. Copy the path of your installed copy of Baldur's Gate 2 into your clipboard. You will need it for the first 4 MODS that install when you execute MegaMoshi.
Now you can began the install!
13. The install will take a very long time and after the first 4 MODS will contiune without interruption for hours. So either surf the net or go watch a movie or something.
14. After it's done you'll need to download the "Game Patch Files" from:

and copy them to the "OVERRIDE" directory in your installed copy of Baldur's Gate 2
15. Download the NOCD patch for Baldur's Gate 2 from here:
http://gf.wiretarget.com/baldurs_gate_2.htm
It patches BGMAIN.EXE
16. Download the Gibberlings 3 WideScreen Mod, extract the files and place them in the Baldur's Gate 2 directory and then execute "setup-widescreen.bat".
Using this MOD was the only way I could use higher resolutions in Baldur's Gate without graphics problems.
15. Run "BGMAIN.EXE" in your Baldur's Gate 2 folder to play Baldur's Gate Trilogy with all of the MODS that you just installed! (Do not use Baldur.exe)
You will need to Insert your "Throne of Bhaal" CD into the CD driver otherwise the game will not launch because it wants the "expansion disc".
NOTES
If when installing the MODS they keep referencing a Baldur's Gate directory that is not specified in SMM then the MODS are reading your local registry. Change your BGMAIN and BGMAIN2 registry keys to the appropriate paths.
