I have an Nvidia Geforce FX5200 and every time the Monitor turns off from inactivity and I reactivate it the screen glitches out and I have to restart the PC for it to return to normal. Has anyone have any Idea what is causing this?

You'll have to try newer and/or older drivers. In my case I could only use latest driver and solution was to disable power saving features in monitor properties, without monitor allowed to go into standby the problem could not manifest.
