I use ms dos6.22 it seems keyboard isn’t work when I plug my keyboard in ps2 port

Or I have to use din5 port on the left only?

No way.
This is an adapter designed for special (ball) mice that support serial and PS/2 protocol.
Note that different adapters with different wiring schemes.
So you need the mouse that was used with this adapter.

But.
There are adapters that convert DIN keyboard port to PS/2 keyboard port.

That DIN keyboard port is electrically identical to PS/2 keyboard port. All you need is a passive adapter, e.g.
