A little guide on how to get it Carmageddon to work with Voodoo 3/4/5.
Example system

The Windows 98 video drivers appear to initialize the card. So you can boot into Windows and play the game, or boot into Windows and then reboot to pure DOS and play. It runs smoother in pure DOS.
I did a full install of the game. With the Max/Splat Pack, the 3dfx.exe that is included works with the Voodoo5 (add the option -vrush to stop HUD flicker). Another exe that works is the Voodoo2 patch from http://www.patches-scrolls.de/ (voodo2c.exe) and this runs without the CD if you did a maximum install.
