As per title, I recently installed a 5.25” drive to my 486DX2 build but my 5.25” drive only able to read and write the 1.2mb disk if I there is a disk in 3.5” drive. Any ideas why its behaving and how to solve this?

Have you tried a different floppy cable?

Show us the cable and where you connect what. If it has the common arrangement as per the picture below, make sure you follow the instructions.
My guess is that you have connected both drives on the same "couple" of connectors.
