I have a Creative Geforce 256 SDR which has severe ghosting/banding but otherwise works without issues.

The drivers install and I can run benchmarks without crashing but there are dark and light shades of horizontal bands across the screen.

I’d like to repair the card if possible but I’m not sure what components to check or replace.
What could cause that issue?

The issue is not the monitor or vga cable and it behaves the same whether the display is connected to the vga or DVI port.

I'm going to guess failing ramdac, I've had a TNT2 look exactly like that. Seeing it would be just a little older then your card it would make sense, I'm pretty sure they would have shared some of the same architecture.

I'm going to guess that if you mess with the resolution the image problem changes slightly, in my case it somewhat lessened as desktop resolution went up.

There were a few threads on here regarding the issue on TNT cards, unfortunately none of them had any successful resolution. I think the general consensus was that it is likely internal thermal damage as nvidias cards at this point in time ran pretty hot.

Is the ramdac a separate chip or is it integrated into the GPU core?

Integrated in the GPU.
