Thanks to Richard Stinchcomb’s tutorial for some tips and tricks.

Install the 16-bit InstallShield Support package for x64 direct from the Microsoft Learn website. It's only a small 152 KB msi file.

After mounting A-10 Cuba CD or iso in Windows 11 25H2 64-bit Windows will complain 16-bit setup.exe is unsupported. Download and extract winevdm OTVDM v0.9 zip from GitHub.

Simply drag and drop the setup.exe from the mounted A-10CUBA onto otvdm.exe the setup will run and install (note I did not have to install OTVDM). I installed the game at the root of C drive under Activison/A-10CUBA.
In game directory
Set A10Cuba.exe properties under compatibility to run as administrator.
For editing text files I recommend Visual Studio Code, I prefer the portable version it’s a zip file.

Add dgvoodoo2 DirectX x86 files to the directory.
https://github.com/dege-diosg/dgVoodoo2
Edit the dgvoodoo config to keep aspect ratio and remove the double cursor.
[General]ScalingMode = stretched_ar[GeneralExt]WindowedAttributes = Borderless, FullscreenSizeFullscreenAttributes = FakeSystemHookFlags = Cursor
You’ll need a different ddraw.dll as dgvoodoo2 version leaves cockpit blurry. Use CNC Ddraw.
https://github.com/FunkyFr3sh/cnc-ddraw
Edit ddraw.dll ini
fullscreen=truewindowed=truemaxfps=-1renderer=opengltoggle_borderless=truesinglecpu=true
Now run the game you’ll get a missing dplay.dll error. Copy "C:\Windows\SysWOW64\dplayx.dll" to game directory and rename it to DPLAY.dll.
If running on a Nvidia GPU add game to Nvidia App and set OpenGL Present Method to Prefer layered on DXGI swapchain.
Now it’s time to BRRRRRRRRRRT!!!!
Keyboard shortcuts
In cockpit hit caps lock to interact with mouse.
A10 Attack (emulated Macintosh) will also run A10 Cuba missions.
