I only have NOLF and the original CD version of it. If I launch the game by NOLF.exe then it crashes for me too, after the initial title screen (fading in/out) before the video playback.
If I start lithtech.exe directly then it mostly works and survives the video playback section.
When it crashes then it crashes in binkw32.dll (the video decoder), it has a memory overrun bug.

I found a lithtech.bat file in my game folder, I probably created it for myself earlier, containing:

lithtech.exe -cmdfile nolfcmds.txt

File nolfcmds.txt is probably generated by the NOLF.exe launcher according to the game settings, it contains the following for me:

LITHTECH.EXE -windowtitle "No One Lives Forever" -rez NOLF.rez -rez NOLF2.rez -rez NOLFdll.rez -rez NOLFl.rez -rez custom +multiplayer 0 +DisableMusic 0 +DisableSound 0 +DisableTripBuf 0 +DisableHardwareCursor 0

Btw, you could use the debug version of dgVoodoo and attach the debug output log to see how far the game gets after it starts.
