I have an EVGA MX 4000 (for some reason marked NV19PL? but it is an MX 4000) with a corrupt vBIOS. It doesn't output anything on one machine, does on the other but makes the computer reboot when booting into Windows.
I've tried looking for PCI version VBIOSes and have found nothing.
I have attached a picture of the card.

Does anybody have a similar card they can share their VBIOS from?

Thanks in advance.

I've got this Jaton MX4000 that I thought was more similar but weirdly this card has Din 9 connector instead of the S-Video of your card and different memory config but still 64bit and 128mb.

Given that I only use VGA, that still might work.

I flashed it and it did take the image, but it seems to not work still. It will POST now but still gives no image (except a white flash a few minutes after starting.) Not sure if it's still a BIOS issue or something else.

Did you make a backup of the original bios? Probably a good idea just to flash it back then just so it's complete.
