I've noticed this problem in almost all DOS games that run under NT based OSes.

With VDMSound et al you can get sfx and midi audio, but cd-rom audio tracks fail to play.

For example Mechwarrior 2, Tomb Raider, Carmageddon etc.

I'm sure someone mentioned an alternative to mscdexnt.exe that fixed this problem.

Can I have a link?

I did a search for sapucdex but only a reference to this forum popped up! 🙄

Do you have any idea at all where I may track down this elusive program?

...and don't forget that you may have to enable or disable Digital CD-audio for your CD-ROM.
