I'm trying to run some old games I played as a kid by using Windows 98 on VirtualBox; I've got to the end of installation and it says "Your computer has already got an operating system installed" and that if I continue the system files will be replaced.

Will it actually replace Windows 7 if I continue, or does this only affect VirtualBox?
Any help is appreciated!

Hmm, this is strange.

Whenever I setup Vbox, I get to create a new VHD to install the VM onto.

Did this not occur ?

Does Virtual Box require that a virtual hard drive be set up beforehand?

That display is from a virtualbox window - see the menu at the top - Machine/View/Devices/Help.

Looks to me like it should be safe - whatever you press with not affect your host OS.

Bit confused by why you say you have reached the end of the install process - have you installed windows 98 and now the VM has rebooted from the 98 cd and it asking effectively about re-installing? In that case just remove the cd and reboot the vm.

Yeah, there was a new VHD made. It has reached the point where it says to restart to continue, then acts as though I'm re-installing. Since last time I have continued and restarted, but it begins the installation again.
I'm a bit confused because when I run the VM it starts the installation again.
I'm using the ISO file, not the disc.

have you checked the vm is set to boot from the vhd? sounds like it is still booting from the iso in which case it will just keep giving you this

You need to change the boot order in Virtualbox. You boot from the ISO to install, once the install is completed you need to boot from the VHD else you will be stuck in endless loop you are in now.

Let me tell you that any game that need direct X will run like shit or not run at all. VirtualBox is a horrible emulator for old PC Games. Heck, im pretty sure you have better luck on DOSBOX itself!
