The icons are 32x32 and 16 colors, but they must match the QuickMenu III default 16-color palette to look right (which is different than the standard Windows 16-color palette).
(Edit) P.S. Here is my own icon collection for QM3 from back in the day. Note that the ZIP contains the file "QM_COLOR.DEF", and also contains all of the stock QM3 icons (as well as all of the custom ones that I had created throughout the years).
