I loved my V1000 Sierra Screaming 3D card. Didn't they need the speedy3d api loaded first or something? I'm not too well versed with these cards so someone else will need to answer this.

It should work if you start it from Win9x. To make Rendition games work within DOS on Verité V2x00 you usually need the tool V2DOSFIX see here.

I used to run D2 on Verite back in the day. In fact I used to have the same card. It's probably installed in something long buried in my storage room.
Fortunately I still *do* have my original batch file to run D2 on the Verite:
set SPEEDY3D_DMA=0
set SPEEDY3D=
d2verite %1 %2 %3 %4 %5 %6 %7 %8 %9 -verbose -playermessages -noredundancy
if exist *.swp del *.swp
Try that!

Don't worry about your card, I get the same image as you have. Don't know maybe it was intended or because the game was written for Verité V1000 and there might be some compatibility problems.
Tomb Raider & IndyCar Racing 2 do have some graphic anomalies that do not happen on the Verité V1000.

Try loading the game without the batch file. V2DOSFIX should be enough.

Just tested on my P200 system with a Diamond Stealth II 220, and I have the same problem.
Seems like getting a Verite V1000 is the way to go.
