Has anyone ever seen this version of the Mach64 VRAM for VLB?

This image is from Canadian computer magazine "HC&E", May 1994 edition. I believe this is a very early version of the Mach64. It could use two types of memory, surface mount or ZIP chips. The card in the photo is fitted with 4MB of surface mount memory (using the upgrade module) while the pads for the zips are empty.
(I believe I actually own 4 or 5 memory expansion modules specifically for this version of Mach64.)
I have seen a few PCI Mach64s that use 2MB of ZIP VRAM, but never an ISA or VLB version.
