You could post a photo of the card... It's easier to look for something than just guessing.
Here is the card. Looks in very good condition. Only some scratches on the back - last picture.
I suggest adding .jpg at the end of URL and than zoom in or save file locally as the resolution is quite big.
If someone needs better picture of some other part of the card, let me know 😉

There are lots of things you can try 😀
Have you tested the card in more than 1 system?
You can:
- Set a wait state on the VLB if the board supports it
- Try other VLB slots
- Remove other VLB cards if present
- Remove the BIOS chip and re-seat it. That fixed one of my cards
Also: If you are overclocking the system, please reset to stock and load BIOS defaults 😀
Thanks for reply... I already tried all the usual things...
1. tested on three different VLB systems
2. tried different VLB slots
3. tried setting 0/1 VLB WS
4. reseated BIOS chip
5. cleaned the contacts
BIOS defaults, no overclocking done whatsoever.
My guess is there is something wrong with the card. But what? Can it be fixed? If not than I'm still on the lookout for one 🙁

Those are some deep scratches, and for the looks of it at least two traces seem to be completly open. You should test the continuity with a multimeter and if it shows open circuit, try to fix them with some small wire, if you're handy with a soldering iron that is.
Have a look at these traces. Somewhere a data bit goes missing: incorrect pixel panning register (text shifted so the first column is missing), and 9th-pixel extension is deactivated (interrupted horizontal text line on the BIOS screen). The scratch is on these data bit traces.
1+1=10
