Would this be an I/O card but what's with the memory? I/O + RAM expansion? Or cache for HDD? If that was a thing for 286(?) or 386(?)

Looks like a RAM expansion board with two serial ports.
I can't decipher the markings on the big chips near the bracket, a better picture of these could probably help.
Proud owner of a Shuttle HOT-555A 430VX motherboard and two wonderful retro laptops, namely a Compaq Armada 1700 [nonfunctional] and a HP Omnibook XE3-GC [fully working :p]

the chips are 82c450's, which are serial port chips (56k).
and two 40 pin ide connectors maybe but I dont see any chips to control them.

Looks like it says 386 SIMM RAM W/2SP, so RAM expansion with 2 serial ports and what looks like two IDE ports

yes looks like 386 SIMM RAM silk screened. Looks like a Wang (or similar) keyboard connector top right of the card, only 5 connections to the Hi bits on the ISA connector so it can not be true 16bit memory interface to the board unless those 40pin connectors go to a proprietary mother/daughter board....not enough parts for IDE controller from 1990.
