So I came across this, most probably a dgVoodoo2 bug, because when I tested it on d3d8to9 and native, it doesn't happen at all, even when forcing AA and AF externally. This also happens with Unreal Engine 2 games, and some RenderWare games as well, like Manhunt 1, I think. Tested with the latest WIP95.3 version.

PS: Nevermind, I managed to fix it. Turns out forced MSAA on dgVoodoo2 causes it, I just had to untick "Force scale&MSAA on RT textures" on the DirectXExt tab, not sure if this fixes the problem entirely, but so far I'm not getting those squares anymore, and still manage to keep forced MSAA on.
