get the latest dosbox from www.dosbox.com and don't use releoaded till game works properly wihout it
and make sure you followed the guide

You are aware that there is no speech with the DOS version, aren't you? If you are looking for more than music, you will need to get the SEGA CD version. Also, as robertmo said, always use the latest DOSBox.
The game's introduction has some digital speech with a SoundBlaster configuration, although the game itself does not, AFAICT. I have no problem getting the speech in the intro to work in DOSBox.
It's been so long that I was forgetting about the intro. But, yeah, make sure that you have SB selected for your audio hardware.
Edit, I have just been playing around with it. It is one of the games affected by the Sierra/Dynamix SB speed bug. Don't start the game at more than about 7000 cycles or the SB driver will not initialize.
