I have tried this on two computers and the Plus "+" Key is the only key not working, I have, set sdl_videodriver=windib, keymapping, keyb us, Removed MS 109 Keyboard from Device Manager, Tested on Win XP 32 bit, w/ Logiteck classic 200 KB & Win 7 Pro 64 bit. w/Logiteck G510 KB. with and without DOSBOX Launcher 074. using DOSBOX 74 + Key doesn't work on DOSBOX command line or while running any games. All other Keys work fine. + key works fine the rest of the time out of DOSBOX.
