Have a Logitech M-SBF96 PS/2 laser mouse and in DOS games (DukeNukem3D) it's really "twitchy" or erratic looking side to side. I boot into DOS
using a PIF file and load the cutemouse driver with a /R1 variable. Anyone know how I might be able to fix this?

The first step is to test with alternate mice to determine if there's a driver issue or a hardware issue. I use Microsoft Optical wheel mice with the USB to PS/2 adapter that came with them on my 486 without any oddities, though I forget if the mouse driver is generic or MS. Definitely not CuteMouse.

Also, try different versions of CuteMouse, because they're different vastly in a way they're handling PS/2 mouses.

Thanks for replying. I deleted the config file within duke3d directory and ran setup again, mouse is working fine now.
