I got this trashed V5 5500 PCI a few years ago, and for another project I needed a V5 with DVI to test some things (my other one literally caught fire.., a repair for another time), so it required some repairs.
It looked something like this:

I first repaired the minimum stuff required for testing: fixed broken traces,replaced the broken/missing resistors. Did not replace any caps, just removed the ones that were cracked and could cause a short. Also added the missing 14.318MHz crystal.
After that I was able to get the master VSA working, so I went on.
Second VSA sort of worked after reflowing it, but in the end I had to replace it. Most likely some of the traces in the substrate were damaged, as some excessive force was applied to it at some point (could see fracture lines under the microscope). When desoldering it, 4 pads also lifted from the PCB. These pads were located in corners, and probably already lifted separated from the PCB while the heatsink was forcefully removed at some point.

Anyway, after replacing all the missing SMDs, fixing traces and pads, new inductors, new crystals, new electrolytic caps, new VSA and some upgrades (128MB RAM, DVI output), here it is:

Ran some 3dmark 2001 SE benchmarks, test system is a 3770K@4.6, 16GB DDR3@2000, Windows XP SP3 with SFFT 1.9 drivers.

Stock card in 33MHz PCI bus: 2904

128MB RAM, core&memory clocked at 184MHz, PCI bus clocked at 95MHz: 3777

I can't push it past 184MHz at the moment, I'll upgrade the cooling (probably design a custom cooling solution for the whole card) and then I'll see if I can get it higher, maybe increase the VSA and RAM voltage a bit.
I should also have some Rev 320 VSAs somewhere, but that will be a last resort, as this PCB has already been through enough.

Very good job! How did you manage to find out what elements are needed for the unpopulated pads for the DVI-output? What camera/postprocessing did you use for these "thermal' images?

You mean that you've unsoldered these SMD components from the other repaired Voodoo5 card one by one to measure their values and then soldered them back to their places?

What thermal camera you're using?

Yes, I measured everything.
I'm also working on this:
Voodoo2 component map + schematics
But for the Voodoo 5 5500 PCI VGA and VGA&DVI versions. Should be ready in a week or so.

The camera is a MobIR 2T. 256x192 pixels, with autofocus.
