Is this GPU dead?

Hi, I just got a 4200 ti and installed it in an Asus a7v400-mx and the fan on the card is very loud and I'm having display problems.

I installed these drivers https://www.vogonsdrivers.com/getfile.php?fil … 408&menustate=0
I followed the hardware wizard but it did nothing. I removed the card and tried the built-in graphics again and those worked fine.
Is this GPU dying or am I an idiot for not setting it up right?

I just ran Hardware info with the card installed and it says AGP 4x @ disabled

AGP does not explain the artifacts. Their regularity suggests broken memory.
