I think the OP's original PSU has Fuhjyyu caps in it. Those capacitors are terrible, and the current draw on CD spin-up probably threw the power out of spec and the system shutdown. You could always crack it open and check if you still have it and care to find out.

Personally, I would reconfigure your soundcard to use DMA 1. Your matching blaster string after resource assignment would be A220 I5 D1 H5 (etc). It is possible to use a configuration (via CTCU/CTCM) that does not enable your "OPL3" functionality. Is 0x388 included in your configuration for the card …

I shipped a CRT to myself (from the city where my parents live) and used FedEx. I told them to pack it fragile, and they used the packing pillows that conform to the shape around them. Arrived nice and snug. But yeah - It was expensive. An alternative and recommended method is to crate it.
