I had a very similar problem with my Biostar 8433UUD. In that case, the cause was a broken address trace. I'm using directly the embedded hard drive host controller. It is working perfectly fine when FSB is set at 66Mhz.

Well in order to use PS/2 port I think it should be enabled first in the BIOS options. When you open your BIOS via MODBIN you'll see that PS/2 mouse support is disabled and hidden from the menu options.
