Actually, looking at the waveform, it looks like it has been processed so that the waveform only has negative amplitude values, I think this might be the trick

Quite interesting, but it seems to me that this has more to do with the wave file instead of the playback code. The wave file is 10khz µ-law with little-endian byte ordering, which seems to sound better when played back using PWM than regular PCM

I'm thinking it's the PCI implementation that could be buggy. Do you have VGA Palette Snoop option in BIOS? Make sure it is set to Disabled. It shouldn't really affect the (S)VGA but I've seen some DACs that are sensitive to write timings, so depending how the snoop is done it might just affect …

Also tried using a SCSI controller/hardrive and disabling the onboard PCI IDE, same thing. I've given up for now and resorted to a PCI video card, but would be good to know if it's the card or the motherboard's bus implementation at fault

Can you try using the BIOS ROM from the other board into this one? Wonder if the BIOS is corrupted or the ROM chip somehow damaged. Have you checked if the traces from the battery to the BIOS are good?

I'd recommend creating a DOS batch file to run under DOSBOX, and then you can trigger it via bash. Don't think you can pass parameters between the OS and DOSBOX, but if you use fixed file names and update them via your bash script, you'll be fine

You can try and make sure that you don't have stuff running in the background, that's... pretty much all you can do. A 486 DX4 100 is an underpowered system to run Windows 98 on, and its overhead becomes more pressing. If you want to try and focus on resolving your pure DOS issues, that might pay …
