Does the USB to IDE adapter have a separate power supply for the 12 V? If not, you can try powering the drive separately from regular PC power supply.

I would partition and format the drive in the 486, then put it in the enclosure and use a modern PC to copy the contents of the setup CD, drivers, etc to it. You can then put the drive back into the 486, boot a win95 floppy and launch the installer from the hard drive. I do this all the time with my …

You could try setting the mode to 0 and see if it helps, if not then it may be a red herring. The CPU speed jumpers are an odd thing to me, but I don't know a ton about VLB. With XTIDE you would only be using the BIOS, not the entire XT-IDE card, and would keep using your existing IDE connectors. It also replaces your INT13H handler, but earlier in the boot process. Can you safely disassemble the computer to verify the connector is installed? That's the first thing I would try before cutting it up. If possible, I would test the keyboard port with the case off, though I doubt Toshiba would have gone through the trouble of populating a non-functioning connector. …

Based on Intel's datasheet, I would guess their reference cooler for a DX2-66 was probably the heatsink with a 40mm fan.
