I think a Voodoo card on a P90 is worthless, i'd say it serves almost 0 purpose on a 486. Also I don't remember Duke 3D working with Voodoo cards.

It's an old game, control expectations weren't really standardized. I think most people probably played this game on Keyboard controls when it came out.

This is the config I settled on for my 4770k. 4.3Ghz (4 Cores) 4.2 Ghz Cache, GTX 690. Under stress it tops out at 73c.

Dunno if you have tried this but I set aspect=false, if I have it set to true I get slowdown. It doesn't seem to effect the way anything looks setting it to false, maybe it just depends on what your monitor does to it on its own? Are you running any programs that limit your FPS like EVGA Precision X? I had a problem that seems similar and it was due to capping my framerate in my main OS.

I've got a dedicated XP Machine: C2D 7300 GTX 280 8 GB RAM It's going to stay that way. I have a modern machine with Windows 8 that I find to be quite nice. I don't get the hate, the Metro interface is really just a fancy Start Menu when you customize it.
