One other bug, very minor but very pesky: Direct3D Fullscreen doesn't erase the host's mouse pointer. At higher resolutions this can be a problem because an entire character can be obscured by the pointer, which isn't movable (nor should it be). Bit of a red herring, surely? Just make the fullscreen toggle something like Ctrl+Alt+Pause/Break, which would rarely if ever be used for anything else.
